What to Expect on Your First Visit
Your complimentary initial appointment will consist of an examination, diagnostic digital photographs and a discussion of potential treatment options. During this visit we will provide an explanation of orthodontics as it pertains to the needs of you or your child. You will also receive an estimate of treatment time and treatment costs. Our objective is to make your visit with us pleasant and informative. Please assist us by providing the following information at the time of your first visit:
- Please fill out and submit our secure online ONLINE PATIENT MEDICAL HISTORY FORM.
- If you prefer, you can download the the form at the link below, fill it out, save to your computer and email it to us at: frontdesk@petalumaortho.com. This fillable form requires Adobe Reader which you can download free here:

- If you have dental or orthodontic insurance please bring your card with you. This will save time and allow us to help you process any claims.
- Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
