During your first visit—which is complimentary—we’ll take all the necessary digital imaging, do a thorough orthodontic exam, and sit down to chat about the best path forward for you or your child. We want you to feel 100% confident in your plan; to help with that, we’ll share the ideal clinical window for the best results, ensuring your treatment is as efficient and effective as possible whenever you’re ready to begin.
Please assist us by providing the following information at the time of your first visit:
- A completed health history form for yourself or your child. Using the links below, please complete all that apply via our secure, HIPAA compliant links:
- ADULTS: Adult New Patient Forms
- CHILDREN: Child New Patient Forms
- If you have dental insurance and/or orthodontic insurance, please inform us prior to your appointment. Providing your insurance details before your visit ensures your treatment plan reflects the most accurate out-of-pocket costs from the start.
